All 8th -12th grade students complete steps 1-4.
All 5th-7th grade students complete steps 1, 3, and 4.
1.) Complete the Online Mission Trip Application and pay the nonrefundable deposit* by April 29th. (*Deposit paid online)

2.) Have an adult complete the Online Adult Reference Form. (8th-12th only)
3.) Print the Medical Release Form and return completed to the HC Office. The Notary Public section will be filled out by Holland Chapel.

D.) Faithfully attend HCSM
E.) Live an existing missional lifestyle
F.) Attend any training or parent meetings
G.) Be ready and willing to share your own testimony
H.) Pray expectantly for our trip
